Ending Homelessness in the Midlands,
One Person at a Time
The journey home begins here. Help us make sure the door stays open.
Real Impact Right Here in the Midlands.
4400+ Individuals placed in permanent housing.
2.7+ Million meals served to those in need.
~50 Community partners working together to provide support.
Behind every number is a person who came to us with nowhere to turn.
A veteran struggling with addiction. A young adult aging out of foster care. A neighbor who was living paycheck to paycheck, you've walked past on Main Street. At Transitions, we meet people where they are and find them a path home.
Transitions doesn't do this alone. More than 50 partner organizations, such as medical providers, legal advocates, job trainers, faith communities, and more, come together under one roof to wrap people in the support they need.
What Happens When Someone
Walks Through Our Doors?
Most people assume we're just a place to sleep for the night. The truth is far bigger than that. When someone walks through our doors, the transition process begins. It starts with safety and ends with freedom and independence.
Engage
We meet people where they are: on the street, in a shelter, or walking through our doors for the first time. No barriers, or prerequisites. Just a safe place to land and a team ready to help.
Equip
Every person gets a case manager, a plan, and access to around 50 community partners. Mental health, job training, legal aid, healthcare, recovery support, and more.
Transition
Transition to permanent housing and real independence. Over 4,400 people have walked out of Transitions and into a home of their own since 2011. We’re more than just a shelter.
Every Homeless Individual in Columbia
Has A Story
At 19, Anastasia had two jobs, nowhere to go, and was alone on a bench in the dark thinking about ending her life. Then Transitions gave her a bed and a case manager who became the mother she never had.
Anastasia's story is one of thousands.

Homelessness in the Midlands Is Closer Than You Think
Most people assume homelessness happens to someone else. The data tells a different story.
South Carolina's poverty rate of 13.9% already exceeds the national average. Half of all renters in this state spend more than 30% of their income on housing. The living wage for a single parent with two children in SC is $43.23 an hour. The minimum wage is $7.25. That gap is where homelessness begins.
These aren't strangers from somewhere else. They're neighbors, veterans, working adults, and seniors who hit a wall that our community hasn't yet torn down.
The problem is real. It's local. And it won't solve itself.
